Dental Forms for Adults, ages 13+
At Spring Lake Dental Group, we have created a business plan that includes receiving health care that keeps your doctor-patient relationship at the forefront while seamlessly involving all our office’s assets in order to reduce confusion, streamline decision making, and make receiving care a comfortable and trusting environment. Our core professional staff, of five general dentists, work as ‘conductors’ in utilizing the talents of specialists in the fields of child dentistry, orthodontics (braces), endodontics (root canal therapy), and periodontics (gum surgeries), as well as hygienists, dental assistants, and patient care coordinators to create a perfect orchestra of talents to deliver modern, preventive health care..
Our practice prides itself on no surprises. Before we begin, you will know what, why, how, and how much. Today’s modern dentistry allows for many options in receiving health care. You need to understand your options in order to make an informed decision. Once we have made you comfortable concerning your decisions, it is up to our individual talents to show you that we can make the experience comfortable, rewarding, and affordable. Even though we have kept ourselves up to date with the latest technology, we realize that our practice has grown since 1996 because we have a special bond with our patients and their care. Patient education, a true concern for your best interests, and a strong doctor-patient relationship will assure quality care.
Please arrive 15 minutes before your scheduled appointment time to allow for patient registration, as we want to spend the appointment time for your treatment. Bring a list of all medications you currently take. Radiographs (x-rays) are adjunctive to the diagnosis process. Forward any existing radiographs to us before your initial visit. Records can be mailed, faxed or emailed to tesha@springlakedental.
